1. We personally know JUAN DELA CRUZ of ______________________, and JANE DELA CRUZ of __________________ City, being an acquaintance for a long period of time;
2. We declare, confirm and affirm that JUAN DELA CRUZ and JANE DELA CRUZ are the biological and lawful parents and/or guardian of JOHN DELA CRUZ who was born on ________________ at ___________________________, Philippines;
3. The JUAN DELA CRUZ that we know and the JUAN DE LA CRUZ indicated in the Certificate of Live Birth of JOHN DELA CRUZ are one and the same;
4. Also, the JANE DELA CRUZ that we know and the JANE DE LA CRUZ  indicated in the Certificate of Live Birth of JOHN DELA CRUZ are one and the same;
5. We are not related by affinity or consanguinity to the above-mentioned persons; and
6. We are executing this affidavit to confirm and affirm the foregoing facts and for whatever legal purpose it may serve.
